By clicking on the "self-test" button below you can access and complete the form designed for the Vancouver Island, BC office. The "Client Form" is not a self-test but provides the Diagnostician with basic information on clients wishing to transfer to our center or who have been Screened and are ready to proceed with lens assessment. These forms are fillable but will need to be saved and emailed to donna@irlenvanisle.com for a free consultation.
